Surgical safety and standards · clot risk assessment and prevention · the complicated and previously operated case
Dr. Shaoliang Wang is Director of the Plastic Surgery & Aesthetics Medical Center at Chongqing West District Hospital and the physician responsible for this site.
His work on this subject is the part of it that happens before an operation rather than after one. He assesses clot risk from the history, the medication, the weight, the smoking status and the expected length of the procedure, and he writes the prevention plan for the operation that is actually planned rather than applying one standard to every patient. He is also the clinician who sees the complicated cases: the patient with a previous clot, the patient on medication that alters clotting, and the patient whose operation has to be shortened or staged because of it. Where the risk cannot be reduced to a level he is prepared to accept, he says so and offers the alternatives rather than proceeding.
Clinical focus
The clot is the complication that planning is for
A clot after surgery is uncommon and it is the reason so much of the preparation for an operation has nothing to do with the operation itself. It forms away from the wound, it can be present with very little to feel, and the treatment that stops it reaching the lung works best when it starts early. That is why the warning signs are taught to every patient rather than to those thought to be at higher risk, and why the plan is written before the anaesthetic rather than after it.
